Cogir Senior Living seeks a Resident Care Coordinator to act as a liaison between residents, families, and the care team, promoting a supportive and safe living environment. They assist the Health and Wellness Director by scheduling, supervising staff, and ensuring systems are in place for quality assisted living services in all resident care areas.

Responsibilities include coordinating care with home health agencies, admissions with the sales team, supervising moves, and maintaining resident

Qualifications

• Two years of experience administering medications and providing care.
• Minimum high school diploma; state medication administration license required.
• LPN/LVN license is a plus.
• Current First Aid and CPR certification or ability to obtain.

Responsabilités

• Schedule and assign caregiving and medication technician shifts to ensure coverage and compliance.
• Recruit and interview new care team members.
• Supervise and train care team; promote teamwork and adherence to standards.
• Oversee medication room and orders in collaboration with technicians and caregivers.
• Maintain accurate resident records, care plans, and incident reports.
• Coordinate on-site home health care and align with policies.
• Coordinate admissions with the sales team; supervise move-ins.
• Conduct family/resident admission interviews and ensure clear communication.
