Job summary

Cogir USA is seeking a Resident Care Coordinator to serve as a liaison among residents, families, and the care team, supporting scheduling, staff supervision, and quality assisted living services across resident care areas.

You will assist with problem-solving resident concerns, coordinate with external providers, and help with medication administration while ensuring care plans and documentation are up to date. Weekend rotation may be required.

Qualifications

• High School Diploma or equivalent is required.
• Valid state medication administration license or meet state requirements for assisted living.
• LPN/LVN license is a plus.
• Current First Aid and CPR license or ability to obtain.

Responsibilities

• Schedule and assign caregiving and medication technician shifts to ensure adequate coverage.
• Participate in recruitment and interview process for new care team members.
• Supervise, train, and support care team members to ensure adherence to care standards.
• Supervise the medication room and orders with the medication technicians.
• Maintain accurate resident records including care plans and incident reports.
• Coordinate care planning with home health agencies on-site per policies.
• Coordinate admissions with the sales team and supervise move-ins.
• Complete family/resident admission interviews.
• Facilitate communication among residents, families, and providers for seamless care.
• Address residents' special needs, requests, and complaints.
• Assist with medication administration and provide direct care when needed.
• Understand community care regulations and state-specific rules.
