Job summary

Cogir Senior Living seeks a Resident Care Coordinator to act as a liaison among residents, families, and the care team to ensure a supportive, safe living environment.

You will assist the Health and Wellness Director with scheduling, supervising staff, and coordinating care with internal and external providers, including medication administration when needed. Weekend rotation is required. Strong leadership and communication are essential.

Qualifications

• High School Diploma or equivalent.
• Must have a valid state medication administration license (such as CNA/HCA) and meet state requirements.
• Nurse Delegation and Diabetes Specialty Certificates, or willingness to obtain within a prescribed period.
• An LPN nursing license is a plus.
• Current First Aid and CPR license or ability to obtain.

Responsibilities

• Schedule and assign caregiving and medication technician shifts.
• Participate in recruitment and interview process for new care team members.
• Supervise, train, and support care team members.
• Supervise the medication room and orders, collaborating with medication technicians and caregivers.
• Maintain accurate resident records including care plans and medical documentation.
• Coordinate care planning with home health agencies on-site.
• Coordinate admissions with the sales team and supervise move-ins.
• Complete family/resident admission interviews.
• Facilitate communication among residents, families, and healthcare providers.
