Job summary

PACS is seeking a Resident Care Coordinator to ensure resident care needs are met under health services leadership. You will coordinate care, supervise health services staff, and assist with scheduling, training and safety protocols.

The role requires flexibility, occasional overtime, and the ability to cover shifts as needed, with a pay range of $25-$28 per hour depending on experience.

Qualifications

• Minimum of one year's experience as a Personal Care Attendant or 6 months as a Medication Aide.
• Experience in an Assisted Living setting preferred.
• Ability to work on-call, weekends and holidays as scheduled.
• Must have and maintain all needed certifications for federal and state requirements (e.g., medication aide, CPR).

Responsibilities

• Partners with health services leadership in developing and updating Cooperative Service Plans.
• Performs resident evaluations as assigned.
• Provides direction and oversight to health services team members.
• Coordinates interviewing, hiring, onboarding and training of new health services staff.
• Maintains medication carts and supplies per physician orders and community policies.
• Coordinates resident transportation and offsite health appointments.
• Acts as a Medication Aide/Personal Care Attendant as needed.
• Educates, monitors and enforces safety and infection control procedures.
