Job summary

PACS is seeking a dedicated caregiver to provide routine daily care and services to residents as outlined in their care plans. The role involves using PCC for accurate resident information, reporting changes to supervisors, and documenting events with care and professionalism.

Responsibilities include assisting with meals, personal care, mobility, and maintaining a safe, respectful, and confidential environment for residents. Prior experience in healthcare is preferred.

Qualifications

• Must possess a high school education or equivalent.
• Ability to read, analyze, and interpret general business material.
• Ability to write reports and communicate clearly with supervisors.

Responsibilities

• Provide routine daily care and services per the resident care plan.
• Use PCC to ensure correct resident information when assisting with ALDS and meals.
• Report changes in residents’ condition to Health and Wellness Director promptly.
• Document entries in notes, SPAs, and incident reports with clear, descriptive language.
• Maintain confidentiality of resident information and report suspected disclosures.
• Assist with mobility and transport within the facility and help residents with personal care.
