Job summary

Avamere at Hermiston in Hermiston, OR is seeking a Resident Care Coordinator (RCC) to coordinate resident care, staffing, and compliance with regulations. The role involves training new staff, supervising care delivery in the director's absence, and maintaining accurate clinical records.

The RCC will assist with daily living activities, documentation, and audits while ensuring a compassionate, professional environment for residents and families.

Qualifications

• Must have an active CPR/First Aid certification.
• High school diploma or equivalent required.
• Prefer a licensed CNA in Oregon.
• Experience with electronic medical records/documentation.

Responsibilities

• Coordinate resident care needs and staffing, maintain schedules and coordination tools.
• Assist Director of Health Services in training new care staff and supervise in their absence.
• Document changes in residents’ status, needs, and appointments; ensure proper care plan documentation.
• Provide daily living assistance and support to residents.
• Schedule Caregivers and Medication Technicians; manage on-call coverage.
• Participate in morning stand-up meetings and Quality Assurance activities.
• Maintain a warm, calm, and cheerful environment; uphold safety and confidentiality.
