Avamere at Newberg is hiring a Resident Care Coordinator (RCC) to document changes in residents’ status, support care plans, and supervise Caregivers and Med Techs. The RCC ensures accurate records, ongoing training, and smooth daily operations in a memory care/assisted living setting.

Responsibilities include guiding new resident assessments and move-ins, maintaining a warm, professional environment, and coordinating with health services for quality care.

Competenze

• Must maintain active CPR/First Aid certification during employment.
• Experience with electronic medical records documentation is preferred.
• High school diploma or equivalent is required.

Mansioni

• Document information on residents’ status, needs, and appointments.
• Ensure documentation of care plans, nurses’ notes, and resident correspondence.
• Provide orientation and on-the-job training to Caregivers and Med Techs.
• Assist with daily living activities (bathing, dressing, eating).
• Conduct weekly, monthly, and quarterly audits.
• Manage scheduling to ensure adequate staff on duty.
• Communicate effectively with residents, families, and staff.
• On-call staffing after hours and weekends as needed.
