Job summary

Avamere Gardens Memory Care in Tigard seeks a Resident Care Coordinator (RCC) to oversee resident care needs and staff scheduling. The RCC supports the Director of Health Services, helps train new staff, and steps in for supervision when needed, ensuring compliance with Avamere policies and regulations.

Responsibilities include documenting resident status changes, updating care plans, and ensuring proper care delivery.

Qualifications

• Caregiver or memory-care experience desired.
• CPR/First Aid certification required.
• Ability to obtain a Food Handler's Card.
• Experience with electronic medical records documentation.
• High school diploma or equivalent.
• Licensed CNA preferred for the state.

Responsibilities

• Coordinate resident care needs and staffing, including schedule management.
• Maintain physician appointment and pharmacy logs; organize clinical records.
• Train new care staff and supervise care delivery when DHS absent.
• Assist residents with daily living activities (bathing, dressing, eating).
• Conduct routine audits and quarterly resident evaluations.
