Job summary

Avamere Skilled Advisors, LLC d/b/a Avamere Living in Hermiston, OR, seeks a Resident Care Coordinator (RCC) to coordinate resident care, staffing, and documentation. This full-time role runs Tuesday through Saturday and supports the Director of Health Services.

The RCC will train Caregivers and Medication Techs, supervise on-call shifts, and ensure compliance with regulations. A caring, confidential approach is essential.

Qualifications

• Caregiving and medication tech experience in memory care/assisted living preferred.
• CPR/First Aid certification required.
• Experience with electronic medical records (EMR) documentation.
• Ability to obtain Food Handler's Card.
• High school diploma or equivalent.
• Licensed CNA preferred in the state of practice.

Responsibilities

• Document changes in residents' status, needs, and appointments.
• Ensure proper documentation of care plans, nurses' notes, and resident communications.
• Provide orientation and on-the-job training to Caregivers and Medication Techs.
• Assist with daily living activities (bathing, dressing, eating).
• Conduct weekly, monthly, and quarterly audits.
• Complete quarterly resident evaluations as assigned.
