Job summary

Avamere at Newberg seeks a Resident Care Coordinator to oversee resident care plans, documentation, and staff coordination in a memory care/assisted living setting. You will ensure accurate charting, conduct audits, and communicate effectively with residents and families.

The RCC will mentor Caregivers and Med Techs, manage staffing, participate in QA meetings, and uphold a warm, compassionate environment while following healthcare standards.

Qualifications

• Active CPR/First Aid certification required.
• Experience with electronic medical records documentation.
• High school diploma or equivalent required.
• Preferred: CNA license in Oregon.
• Must be 18 years or older.

Responsibilities

• Document changes in residents’ status, needs, and appointments.
• Ensure proper documentation of care plans, nurses' notes, and resident correspondence.
• Provide orientation and on-the-job training to Caregivers and Medication Techs.
• Assist with daily living activities (bathing, dressing, eating).
• Conduct weekly, monthly, and quarterly audits and evaluations.
• Schedule Caregiver and Medication Tech staff to ensure on-duty coverage.
• Participate in morning stand-up meetings and QA activities.
• Create and maintain a warm, patient, enthusiastic and calm environment.
