Job summary

Avamere at Chestnut Lane in Gresham, OR is seeking a Resident Care Coordinator (RCC) to coordinate resident care needs and staffing, maintain schedules and logs, and support the Director of Health Services in staff training. The RCC will supervise in the Director’s absence and ensure compliance with state and federal regulations, while fostering a warm, professional atmosphere for residents.

The role includes on-call responsibilities after hours and weekends, and requires weekend availability as

Qualifications

• Must have a high school diploma or equivalent.
• CPR/First Aid certification active during employment.
• Experience as a Caregiver or Medication Tech preferred in Memory Care/Assisted Living.
• Ability to obtain a Food Handler’s Card.

Responsibilities

• Coordinate resident care needs and staffing, including scheduling and logs.
• Assist Director of Health Services in training new care staff.
• Supervise resident care services in the Director’s absence and ensure regulatory compliance.
• Document changes in residents’ status, needs, and appointments.
• Conduct audits (weekly, monthly, quarterly) and participate in QA meetings.
• Manage caregiver and medication staff scheduling to ensure adequate coverage.
• Provide orientation and on-the-job training to staff.
• Support daily resident care, communication, and problem-solving.
