Job summary

Willow Creek Healthcare Center seeks a compassionate, organized, and dependable Non-Licensed Case Manager to join our team. This position does not require a professional license and involves coordinating care from admission through discharge.

The ideal candidate has strong communication and critical-thinking skills, thrives in a fast-paced healthcare environment, and is committed to providing an exceptional resident experience.

Qualifications

• Bachelor's degree preferred in social work, human services, healthcare administration, nursing, or related field.
• Experience in healthcare, case management, admissions, discharge planning, social services, or skilled nursing strongly preferred.
• Knowledge of Medicare, Medi-Cal, Medicaid, insurance benefits, and authorization processes preferred.
• Experience with EMR systems preferred.
• Certified Case Manager credential is a plus but not required.
• Strong communication, organization, documentation, and time-management skills.

Responsibilities

• Coordinate care from admission through discharge.
• Communicate plans of care with residents and families.
• Attend daily meetings to support communication and coordination.
• Coordinate referrals to and from the facility.
• Perform utilization review to support appropriate, timely, and cost-effective care.
• Maintain accurate documentation and complete required paperwork within established timeframes.
• Protect residents' privacy and maintain confidentiality of health information.
• Develop organized work processes and adjust priorities in a busy environment.
• Maintain professional and respectful relationships with residents, families, staff, and partners.
