Job summary

DESERTWILLOW seeks a Case Manager (Non Licensed) to coordinate patient care across residents, families, providers, and payers. You will evaluate needs, develop plans, and monitor progress to deliver high-quality, cost-conscious care.

Ideal candidates hold a Bachelor's in Social Work or related field, with healthcare or case management experience. You will document thoroughly, communicate clearly, and work collaboratively in a fast-paced setting.

Qualifications

• Bachelor's degree in Social Work, Human Services, Nursing, or related field preferred (non-licensed candidates accepted).
• Prior healthcare, case management, or social services experience preferred.
• Knowledge of Medicare, Medi-Cal, and Medicaid programs and benefits.
• Familiarity with PCC (PointClickCare) or EMR systems preferred.

Responsibilities

• Communicate regularly with residents and families about plans of care and treatments.
• Attend and participate in meetings to facilitate team communication.
• Monitor and adjust residents' statuses based on changing needs.
• Complete documentation as required and perform utilization review activities.
• Coordinate care with residents, providers, and payers.
• Oversee admissions and discharge activities and coordinate referrals.
