Qualifications

• Healthcare administration experience preferred.
• Excellent written and verbal communication.
• Ability to manage multiple priorities in a fast-paced environment.
• Experience gathering medical and social history for resident profiles.

Responsibilities

• Coordinate the admission process for new residents, including gathering and reviewing documentation, conducting tours, and introducing residents to our care team
• Work closely with families and healthcare professionals to gather medical and social history information to create a comprehensive patient profile
• Collaborate with health care teams to ensure accurate and timely communication of resident information and needs
• Maintain accurate and up-to-date records of resident admissions, transfers, and discharges
• Develop relationships with healthcare professionals, social workers, and other referral sources to ensure a steady flow of referrals
• Provide exceptional customer service to residents, families, and healthcare professionals
