Job summary

Willow Creek Healthcare Center is seeking a compassionate, organized Non-Licensed Case Manager to coordinate resident care from admission to discharge. The role does not require a professional license and involves collaborating with families, providers, and payers to ensure quality services.

Qualifications include a bachelor’s degree in a related field and strong communication skills. Experience in healthcare case management and familiarity with EMR systems is preferred.

Responsibilities

• Coordinate care from admission through discharge with residents and families.
• Attend daily meetings to support interdepartmental communication.
• Maintain documentation and ensure timely completion of paperwork.
• Coordinate referrals and utilization review to ensure cost-effective care.
• Communicate with physicians, nurses, therapists, payers, and care teams.
