Qualifications

• Knowledge of Medicare, Medi-Cal, and Medicaid programs and benefits.
• Familiarity with electronic medical record systems (e.g., PCC) preferred.
• CCM certification a plus.

Responsibilities

• Coordinate resident care with families, providers, and payers.
• Communicate plans of care and treatment updates to residents and families.
• Attend team meetings and facilitate communication among staff.
