Job summary

HealthMap Solutions is seeking a Registered Nurse, Care Navigator/Case Manager to lead case management for kidney health, coordinating care and improving outcomes for a mixed caseload. You will document in Compass, perform health screenings, reconcile medications, and maintain patient-centered care plans.

Ideal candidates will have an active Oregon RN license and 6+ years of RN experience including 3+ years in case management. CCM and dialysis/transplant experience are pluses.

Qualifications

• Active, unrestricted RN license required.
• Bachelor’s degree required; 6+ years of RN experience including 3+ years in case management may be considered in lieu of degree.
• CCM preferred.
• Three (3) years of experience in case management preferred.
• Experience in a dialysis center or transplant center preferred.
• Experience with Medicare and Medicaid preferred.

Responsibilities

• Handle in and outbound calls delivering world-class service to our members.
• Educate kidney health and related comorbid conditions and optimize renal replacement therapy.
• Engage members into HealthMap’s Kidney Health Program.
• Follow up with members based on complexity and cadence by policy.
• Serve as patient advocate for responding to concerns or barriers.
• Utilize community resources and programs in care planning.
• Coordinate care between patient, support network, physician, and other providers as part of an interdisciplinary team.
• Notify providers of identified patient needs.
• Comply with HIPAA and all regulations.
• Comply with company policies, procedures, and security policies.
• Be accountable for individual metrics and KPIs.
