Job summary

U.S. Renal Care seeks a Value-Based Care Nurse Case Manager to support chronically ill kidney disease patients across Texas clinics. The role involves education, medical history review, medication coordination, and building support networks to reduce costs and facilitate dialysis or transplant planning.

Qualifications

• Must hold Texas RN license and be in good standing.
• Three+ years of nursing experience preferred.
• Experience in case management and/or providing nursing care to dialysis patients preferred.
• English communication; Spanish/Bilingual a plus.
• Proficient with USRC clinical applications within 90 days of hire.

Responsibilities

• Case management activities: assess, coordinate, monitor, and evaluate care.
• Provide health education, coaching, and treatment decision support for patients.
• Collaborate with patient, caregivers, and other healthcare providers to address patient needs.
• Build partnerships with nephrologists and dialysis team to manage care.
• Drive metrics that improve patient outcomes and reduce hospitalizations and readmissions.
