Qualifications

• Active RN license required; compact license preferred.
• Experience in care management for patient-centered programs.
• Proficient with MS Office; comfortable with Outlook/Teams.
• Ability to work effectively in a remote environment with a dedicated workspace.

Responsibilities

• Manage an assigned patient panel with care coordination.
• Build trusting relationships with patients and families.
• Advocate for patients and navigate the health system.
• Develop patient-centric care plans from evaluations and goals.
• Coordinate interdisciplinary care to reduce fragmentation.
• Monitor outcomes and adjust interventions as needed.
• Follow evidence-based care management guidelines and workflows.
• Collaborate with physicians, APPs, and IDTs on integrated plans.
• Identify cost-effective measures supporting value-based care.
• Educate patients and caregivers on treatment options.
• Support seamless transitions of care across settings.
• Participate in clinical huddles and care conferences.
• Prepare reports on program success and patient satisfaction.
• Engage in process improvement to optimize workflows.
