Job summary

Healthmap Solutions is seeking a Clinical Care Manager/Clinical Nurse Liaison in Fayetteville, NC to develop long-term relationships with physicians and Healthmap members enrolled in the Kidney Management Program. The role coordinates care management and performance improvement activities to improve health outcomes.

The ideal candidate has a registered nurse license, 6+ years of RN experience (3+ years in case management), and willingness to travel up to 15% within the assigned region.

Qualifications

• Bachelor’s degree required; 6+ years of RN experience, 3+ years in case management may substitute for degree
• Active, unrestricted RN license required
• BLS certification required for all field roles (within 30 days of hire)
• 3+ years of healthcare services, clinical operations, quality, or care management
• 3 years in care gap closure or coordination in outpatient or hospital settings preferred
• Proof of valid driver’s license; regular travel within assigned region
• Must reside in one of the assigned states

Responsibilities

• Act as liaison between Healthmap, provider practices and Healthmap members to ensure engagement and performance with programs
• Develop provider/partner relationships to improve provider and member engagement and manage caseload
• Identify opportunities to improve health outcomes based on provider data
• Educate on Best Practice sharing, process improvement in provider workflows and NCQA measures
• Educate providers on Chronic Kidney Disease and related topics
• Partner with physicians to identify Healthmap members who would benefit from Care Navigation
• Educate Healthmap members on kidney health and related conditions
• Serve as member advocate and coordinate with treating providers to meet goals
• Be accountable for metrics and KPIs
• Ensure timely delivery of reports to stakeholders
• Maintain documentation in compliance with NCQA and HIPAA
• Follow Healthmap policies and procedures and regulatory requirements
• Perform other related duties
