Job summary

A healthcare organization is seeking a remote RN to manage patient care related to chronic kidney disease. Responsibilities include ensuring continuity of care, coordinating treatment plans, and documenting patient information. Candidates must have an active RN license and 2+ years of nursing experience, preferably in dialysis care. Flexible evening hours and a supportive work environment are provided.

Qualifications

• Active RN license in current state of residence with ability to qualify for additional state licenses.
• 2+ years of nursing experience in a hospital, acute care, or direct care setting.
• Renal, Chronic Kidney Disease or Dialysis Care experience as a main focus.

Responsibilities

• Ensure continuity of care in both inpatient and outpatient settings.
• Consult with team members to ensure care plans are implemented successfully.
• Document patient information and care plans in Electronic Health Record.
