Job summary

Intermountain Health seeks a skilled registered nurse to lead specialty care management within the Intermountain system, coordinating with patients, families, physicians, and care teams across the care continuum. The role emphasizes evidence-based planning, advocacy, and resource coordination to meet patient goals and promote engagement.

The position requires five years of direct patient care, a current RN license, and BLS, with a BSN to be obtained within three years of hire.

Qualifications

Current Registered Nurse license in the state you practice.
Basic Life Support (BLS).
Five years of direct patient care experience.
BSN within three years of hire or promotion (BSN not required for Nevada positions).

Responsibilities

Apply Case Management Standards of Practice (Assess, Plan, Facilitate and Advocate) to manage care for assigned patient population.
Assess patient/family understanding of diagnosis and health management goals, collaborate with physicians and team members.
Plan with patient/family and health care team to develop a treatment plan improving outcomes.
Facilitate communication between family and team members.
Advocate to resolve barriers to care using available resources and processes.
Document interactions and interventions; participate in team conferences and tracking.
