Job summary

Somatus is seeking an RN Care Manager to focus on high-needs Chronic Kidney Disease patients in Portland, Oregon. This role emphasizes collaboration with physicians to enhance patient care, including face-to-face visits and telehealth consultations.

The ideal candidate holds an RN license and has experience in renal care as well as a strong commitment to patient-centered service. The position includes diverse responsibilities in care coordination and building trusted relationships with patients and providers.

Qualifications

• 2+ years of RN experience, including multidisciplinary care.
• Valid BLS certification from an AHA or American Red Cross provider.
• Renal or Chronic Kidney Disease experience is essential.

Responsibilities

• Conduct face-to-face visits with patients and physicians.
• Collaborate with patient advocates and interdisciplinary teams.
• Provide care coordination services through telehealth or in-person.
