Job summary

Qualifications

• Minimum of 2 years experience in clinical renal patient care.
• Experience with renal transplant, dialysis, or CKD care preferred.
• Understanding of diabetes and cardiovascular disease processes preferred.

Responsibilities

• Educate patients and families about CKD treatments and options.
• Coordinate care with nephrology staff and track patient progress.
• Assist in arranging dialysis access and transitions to RRT.
