Qualifications

• Minimum of 2 years previous experience in clinical renal patient care.
• Combination of renal transplant, dialysis, or CKD patient care.
• Understanding of diabetes and cardiovascular disease processes preferred.
• Strong organizational and communication skills.

Responsibilities

• Assess patient knowledge of late-stage CKD and treatments; educate and guide decisions as patients transition to RRT.
• Coordinate care and support for patients seeking conservative or palliative options.
• Liaise with staff to provide information on treatment modalities, HD access types, and safety benefits.
• Educate CKD patients on education topics, modality selection, access placement, and RRT transition.
