Job summary

Interwell Health is a kidney care management company dedicated to reimagining healthcare and helping patients live their best lives. The Renal Care Coordinators will be embedded within a local nephrology practice in Reno/Carson City, NV, onsite, with light travel to nearby satellite locations.

A one-time $5,000 sign-on bonus is available. As an RCC, you will educate CKD patients, coordinate care across modalities, and support transitions to RRT, including hemodialysis and peritoneal dialysis.

Qualifications

• Minimum of 2 years previous experience in clinical renal patient care.
• Combination of renal transplant, dialysis, or CKD patient care required.
• Understanding of diabetes and cardiovascular disease processes preferred.
• Strong organizational and communication skills.

Responsibilities

• Assesses patient knowledge of late-stage CKD and educate patients for informed decisions during transition to RRT.
• Liaises with staff to ensure patients receive comprehensive information on modality options and dialysis alternatives.
• Organizes CKD education and access placement for stable transition to RRT.
• Participates in interpretation of summary clinical data to improve CKD care processes.
