Job summary

Interwell Health is hiring Renal Care Coordinators (RCCs) to support late-stage CKD patients through education, care coordination, and modality discussions. This onsite role in Reno/Carson City, NV may involve travel to nearby satellite sites and includes a $5,000 sign-on bonus.

Two years of renal care experience is required. RCCs collaborate with nephrology teams to optimize transitions to RRT, provide patient education on dialysis options, and coordinate care plans across settings with

Qualifications

Minimum of 2 years of experience in clinical renal patient care.
Combination of renal transplant, dialysis, or CKD patient care required.
Understanding of diabetes and cardiovascular disease processes preferred.

Responsibilities

Assesses patient knowledge of late-stage CKD and treatments, educating patients to enable informed decisions and care coordination.
Liaises with staff to ensure patients receive information on modality options, hemodialysis at home/in-center, peritoneal dialysis, transplantation, and conservative care, emphasizing AV fistula or AV graft benefits.
Organizes the nephrology practice CKD education including modality selection, access placement and maintenance, and transition to RRT.
Participates in interpretation of summary clinical data to improve late-stage CKD care processes.
