Job summary

Interwell Health is seeking a Renal Care Coordinator for an onsite role in the Reno/Carson City area of Nevada. You will coordinate care for late-stage CKD patients, educate families, and liaise with staff to present modality options including home dialysis and transplantation.

A one-time $5,000 sign-on bonus is offered and some travel to satellite locations is expected. The position emphasizes education, care coordination, and collaboration with nephrology teams to improve patient outcomes

Qualifications

• Minimum 2 years renal patient care experience.
• Experience with renal transplant, dialysis, or CKD patient care.
• Strong organizational and communication skills.

Responsibilities

• Assess patient knowledge of late-stage CKD and educate patients for informed decisions during transition to RRT.
• Serve as liaison to ensure patients and families receive information on modality options and dialysis methods (home and in-center) including AV fistula/graft benefits.
• Organize CKD education, modality selection, access placement, and transition to RRT within the nephrology practice.
• Interpret summary clinical data to improve late-stage CKD care processes.
