Job summary

Interwell Health is seeking a Renal Care Coordinator to organize care for late-stage CKD patients within a local nephrology practice. You will provide education, support, and care coordination to help patients begin dialysis, in a full-time onsite role based in Takoma Park, Maryland, with possible light travel to nearby satellite locations.

The role requires at least 2 years of renal patient care, experience with dialysis or CKD, and strong organizational and communication skills.

Qualifications

• Minimum of 2 years of renal patient care experience.
• Experience with dialysis or CKD patient care.
• Understanding of diabetes and cardiovascular disease processes preferred.
• Strong organizational and communication skills.

Responsibilities

• Assess patient knowledge of late-stage CKD and treatments; educate and inform patients for informed decisions during transition to RRT.
• Act as liaison to provide comprehensive information on modality options and care plans including HD/PD and transplantation.
• Organize CKD education and permanent access placement and maintenance; facilitate a stable transition to RRT.
• Interpret summary clinical data to improve late-stage CKD care processes.
