Responsibilities

• Assess patient knowledge of late-stage CKD and treatments, educating patients to enable informed decisions during the transition to Renal Replacement Therapy (RRT).
• Provide support, guidance, and coordination of care for patients seeking conservative or palliative care.
• Act as a liaison with staff to ensure comprehensive information on modality advantages and disadvantages, hemodialysis treatments, transplantation, and conservative care.
• Organize CKD education within the nephrology practice, including modality selection and stable transition to RRT.
