Job summary

Interwell Health is seeking Renal Care Coordinators to support late-stage CKD patients within a nephrology practice. You will educate patients, coordinate care, and liaise with the care team to optimize dialysis decisions and transition planning.

You will work onsite in Reno/Carson City, NV, with some light travel to nearby satellite locations. The role requires at least 2 years in renal patient care and strong organizational and communication skills.

Qualifications

• Minimum of 2 years experience in clinical renal patient care.
• Combination of renal transplant, dialysis, or CKD patient care required.
• Understanding of diabetes and cardiovascular disease processes preferred.
• Strong organizational and communication skills.

Responsibilities

• Assesses patient knowledge of late-stage CKD and treatments; educates patients to enable informed decisions about dialysis transitions.
• Acts as liaison with staff to ensure comprehensive information on modalities, hemodialysis, PD, and transplantation; emphasizes AV fistula or graft benefits.
• Organizes CKD education and modality selection, permanent access placement, and transition to RRT.
• Interprets summary clinical data to improve late-stage CKD care processes.
