Job summary

Interwell Health is seeking a Renal Care Coordinator (RCC) to support late-stage CKD patients within a local nephrology practice. You will educate patients, coordinate care, and discuss modality options to ensure a smooth transition to renal replacement therapies (RRT).

The role is full-time and onsite in Reno/Carson City, NV with light travel to nearby satellite locations. A $5,000 sign-on bonus is available, and the team emphasizes collaborative, patient-centered care.

Qualifications

• Minimum of 2 years experience in clinical renal patient care.
• Experience with renal transplant, dialysis, or CKD patient care required.
• Strong organizational and communication skills.

Responsibilities

• Educate CKD patients about treatment options and support transition to RRT.
• Liaise with staff to provide education on treatment modalities and AV fistula/graft benefits.
• Coordinate education and access planning for CKD patients preparing for RRT.
• Interpret summary clinical data to improve late‑stage CKD care processes.
