Job summary

Interwell Health in Longview, TX is seeking a Renal Care Coordinator (RCC) to support late-stage CKD patients by coordinating care, education, and liaison with nephrology teams. This full-time onsite role offers impactful patient support and a one-time sign-on bonus.

You will educate patients on dialysis options, arrange care transitions, and work with staff to ensure timely, safe treatment decisions. Light travel to satellite locations may be required; interview details discussed.

Qualifications

• Minimum of 2 years previous experience in clinical renal patient care.

Responsibilities

• Assesses patient knowledge of late-stage CKD and treatments; educates and informs patients to enable informed decisions during transition to RRT.
• Acts as a liaison with staff to ensure patients and families receive information on modality advantages and disadvantages, dialysis options (home and in-center), peritoneal dialysis, transplantation, and AV fistula vs CV catheter implications.
• Organizes CKD education and access planning for stable transition to RRT; coordinates permanent access placement and maintenance.
• Participates in interpretation of summary clinical data to improve late-stage CKD care processes.
