Job summary

Interwell Health in Longview, Texas is seeking a Renal Care Coordinator (RCC) to join our team onsite. The RCC will organize care for late-stage CKD patients, providing education, care coordination, and support through the transition to renal replacement therapy.

This is a full-time onsite role with possible light travel to nearby locations. The role emphasizes patient education, coordination with nephrology teams, and ensuring patients and families receive comprehensive information about

Qualifications

• Minimum of 2 years experience in clinical renal patient care.
• A combination of renal transplant, dialysis, or CKD patient care required.
• Understanding of diabetes and cardiovascular disease processes preferred.
• Strong organizational and communication skills.

Responsibilities

• Assesses patient knowledge of late-stage CKD and treatments, educating and informing patients to enable informed decisions during transition to RRT.
• Acts as liaison with staff to ensure information on modality advantages and disadvantages, hemodialysis at home and in-center, peritoneal dialysis, kidney transplantation, and conservative care is provided; education on AV fistula or AV graft vs central venous catheters.
• Organizes the nephrology practice late-stage CKD patient population regarding education, modality selection, permanent access placement and maintenance, and a stable transition to RRT.
• Participates in interpretation of summary clinical data and its use in improving late-stage CKD care processes.
