Job summary

Interwell Health in Baltimore, MD, is seeking Renal Care Coordinators to support late-stage CKD patients with education, care coordination, and transition planning to dialysis. This is a full-time, onsite role based in Baltimore, with potential light travel to nearby satellite locations.

The RCC will educate patients on dialysis modalities, coordinate with nephrology staff, and help organize access placement and maintenance to ensure a smooth transition to renal replacement therapy.

Qualifications

• Minimum of 2 years previous experience in clinical renal patient care.
• Combination of renal transplant, dialysis, or CKD patient care required.
• Understanding of diabetes and cardiovascular disease processes preferred.
• Strong organizational and communication skills.

Responsibilities

• Assess and educate late-stage CKD patients to support decisions during transition to RRT.
• Coordinate information about modalities (hemodialysis at home/in-center, peritoneal dialysis, transplant, conservative care) and vascular access options.
• Organize CKD education and permanent access placement within the nephrology practice.
• Interpret summary clinical data to improve late-stage CKD care processes.
