Job summary

UnitedHealth Group is seeking Wellness Coordinators to provide care coordination and administrative support to members with behavioral health and substance use needs. This non-clinical role engages members, screens for gaps in care, and helps develop individualized care plans.

The position is remote nationwide, 40 hours/week, with standard business hours; the team offers comprehensive benefits and opportunities for growth within UnitedHealth Group's behavioral health services.

Qualifications

• High School Diploma is required.
• 2+ years of experience in behavioral health and case management.
• 1+ years of computer proficiency.
• 1+ years of dual diagnosis experience with mental health and substance abuse.
• 1+ years of experience handling sensitive issues with members and providers Confidential manner.
• Access to a quiet workspace at home with PHI security.
• Must have reliable high-speed internet.
• Ability to work 40 hours in PST or CST.

Responsibilities

• Manage inbound and outbound member queue.
• Document in electronic systems.
• Coordinate information with clinical team partners.
• Provide behavioral health benefit education.
• Contact members to screen for gaps in care.
• Provide network referrals and community resources.
• Provide administrative support.
• Perform other duties as assigned.
