Job summary

Actalent is seeking a compassionate Behavioral Health Coordinator to assist members in navigating behavioral health and substance use treatment services. You will connect individuals with providers, coordinate care, and perform outreach to keep members engaged and supported throughout their treatment journey.

The role emphasizes remote work, strong communication, and collaboration with providers, social workers, and community resources to improve outcomes for underserved populations.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, Public Health, Healthcare Administration, or a related field.
• 2+ years of experience in behavioral health, care coordination, case management, managed care, or social services.
• Knowledge of mental health and substance use resources.
• Strong communication, organization, and relationship-building skills.
• Proficiency with Microsoft Office and electronic documentation systems.

Responsibilities

• Connect members with behavioral health providers and assist with appointment scheduling.
• Conduct outreach and follow-up with members facing barriers to care.
• Educate members on available benefits, treatment options, and community resources.
• Coordinate care with providers, social workers, and community organizations.
• Facilitate referrals and ensure timely access to services.
• Connect members with support resources, including transportation, housing, and social services.
• Monitor member engagement and maintain accurate documentation.
• Manage caseloads and prioritize follow-up activities.
• Collaborate with internal and external partners to improve member outcomes.
