Addus Homecare is seeking a Full-Time Home Health Weekend Intake Coordinator to join our remote team serving the New Mexico market. The role requires managing referrals from intake to completion, ensuring complete documentation and timely admission during weekend shifts (Fri 8am-6pm, Sat-Sun 8am-8pm, Mon 8am-6pm).

You’ll collaborate with referral sources, clinicians, and scheduling teams to ensure a smooth transition into care, while maintaining compliance and excellent patient service.

Competenze

• High school diploma or equivalent required; further education preferred.
• Experience in home health intake, healthcare coordination, or patient access is a plus.
• Knowledge of home health regulations and Medicare guidelines is beneficial.

Mansioni

• Manage assigned referrals from receipt through completion and readiness for scheduling.
• Review referrals for completeness, accuracy, and eligibility in line with home health guidelines.
• Ensure all required documentation is obtained from portals and external sources for a complete referral package.
