Ambercare – an Addus family company in Santa Fe, NM is seeking a Full-Time Home Health Intake Coordinator to manage referrals from intake to admission, ensuring complete documentation and timely processing. The role collaborates with referral sources, patients, clinicians, and scheduling teams to ensure a smooth transition into care.

Local candidates only; Monday–Friday 8am–5pm MST, on-site in the Santa Fe area.

Qualifications

• High school diploma or equivalent required.
• Associate or Bachelor’s degree preferred.
• Experience in home health intake, healthcare coordination, or patient access is a plus.
• Knowledge of home health regulations and Medicare guidelines is helpful.
• Proficiency in EMR systems and Microsoft Office.

Responsabilités

• Manage assigned referrals from receipt through completion and readiness for scheduling.
• Review referrals for completeness, accuracy, and eligibility per home health guidelines.
• Ensure all required documentation is obtained from referral portals and external sources.
• Review clinical documentation to confirm it meets regulatory and organizational requirements.
• Upload and organize clinical records in the appropriate systems.
• Follow processes to obtain missing or incomplete documentation in a timely manner.
• Verify that MD orders will be signed prior to admission.
• Perform welcome calls to patients/caregivers and set expectations.
• Assign disciplines to referrals based on clinical needs and physician orders.
• Notify scheduling when referrals are complete and ready for visit scheduling.
• Explain benefits to patients, families, and community contacts.
• Monitor referral progress to avoid delays in care.
• Enter delay notes for referrals not submitted on time.
• Follow up on outstanding items to move referrals toward admission.
• Serve as liaison between referral sources, patients, physicians, and internal teams.
• Maintain clear communication on referral status and requirements.
• Collaborate with Intake Analysts, clinicians, and scheduling teams for continuity.
• Maintain detailed records of referral activity and status updates.
• Ensure all documentation is filed and accessible for audits.
