Job summary
Canyon Home Care and Hospice seeks a skilled Registered Nurse to support Intake and Admissions. You will process referrals, verify eligibility, and complete admission assessments, ensuring timely start of care, especially during weekend rotations.
Responsibilities include in-home or virtual assessments, comprehensive nursing evaluations, documentation in EMR, and coordinating care with the IDT. Weekend coverage and independent judgment are essential.
Qualifications
- Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
- One year nursing experience preferred.
- Acceptance of philosophy and goals of this Agency.
- Ability to exercise initiative and independent judgment.
Responsibilities
- Receives and processes new referrals; verify insurance and service eligibility.
- Complete in-home or virtual admission assessments.
- Perform comprehensive clinical evaluations and medication reconciliation.
- Document accurately and timely in the EMR.
- Coordinate with providers, schedulers, and interdisciplinary team for smooth transitions.
- Initiates the plan of care and necessary revisions.
- Supervises LPN/LVN and home health aides per schedule.
- Completes OASIS assessments and discharge summaries as required.
