Job summary

CVS Health seeks a licensed Family Nurse Practitioner to provide remote care through a virtual platform. This weekend/evening, full-time role requires independent patient management, collaboration with a care team, and documentation in the EHR.

Ideal candidates hold an active CT/GA/NC/ND/NJ/NY/PA/SC/SD/VA/WI/WV license and a Master’s in Family Nurse Practitioner with board certification. Comprehensive benefits are offered.

Qualifications

• Licensed in CT/GA/NC/ND/NJ/NY/PA/SC/SD/VA/WI/WV to be considered
• Master’s degree in Family Nurse Practitioner with current certification
• Experience and capability to provide autonomous patient care

Responsibilities

• Provide holistic, evidence-based care including assessment, diagnosis, and treatment
• Evaluate primary, acute, chronic, and transitional care patients aged 18 months and older
• Counsel on pregnancy prevention, STI prevention, contraceptive care, and meds management
• Educate patients on health maintenance and respond to inquiries
• Document care in the EHR per MinuteClinic policies
• Coordinate care with internal and external colleagues to ensure high-quality care
• Collaborate with Providers, physicians, and health care team members
• Work independently, prioritize tasks, and advocate for patients
