Job summary

CVS Health is seeking a Minute Clinic Virtual Care Nurse Practitioner (Provider) to deliver patient care via a remote technology platform. The role requires collaboration with a dedicated team to provide holistic, evidence-based care for patients age 18 months and up, including pregnancy prevention and STI counseling.

Qualifications

• Two years of medically-relevant experience or equivalent.
• Active licensure in AL, AR, CO, IA, ID, IN, LA, MI, MS, NE, NV, OK, TN, or TX.

Responsibilities

• Provide holistic, evidenced based care including assessment, diagnosis, treatment, management of health problems, health counseling and disposition planning for patients 18 months and older.
• Evaluate primary care, acute and chronically ill patients and provide healthcare education and counseling.
• Document all patient care within an EHR per MinuteClinic policies.
• Coordinate care with internal and external colleagues to ensure high-quality care.
• Work with a care team and advocate for patients and the practice.
