The role involves interviewing, assessing, counseling, and developing comprehensive plans to support aging in place across MD, DC, and VA. Responsibilities include guiding clients through options, coordinating services, and ensuring effective case management.

Qualifications

• Bachelor’s Degree in social work, human services, or related field.
• Strong interviewing, oral and written communication skills.
• Proficient use of computers, software, and mobile technology.

Responsibilities

• Advise veterans and caregivers on services to support aging in place.
• Conduct interviews to screen and assess needs.
• Provide options counseling and develop care/service plans.
