Job summary

Bay Aging seeks an Options Counselor to provide person-centered counseling to Veterans in the Veterans Directed Care program. You will assess needs, develop spending plans, and coordinate supports with the Veteran and service providers.

Responsibilities include home visits, office follow-ups, record-keeping, and monthly expenditure reporting. Remote work is common, with travel to beneficiary homes and local offices as needed.

Qualifications

• 3+ years of experience working with seniors, disabled adults, human services or case management fields.
• Bachelor’s degree in social work, human services or medical field preferred.
• Criminal History background check required.
• Experience with HIPAA guidelines and confidentiality best practices is preferred.

Responsibilities

• Perform home visits and assess veterans' needs, develop spending plans, and support employer setup (up to 40 veterans).
• Maintain client records, monitor expenditures, and generate monthly reports.
• Travel up to 50% and manage office tasks, referrals, and data entry in databases.
