Peninsula Agency on Aging seeks an Options Counselor for the Veteran’s Directed Care Program in North Carolina. The role focuses on advising veterans and caregivers about available services, completing assessments, and developing care plans.

Qualifications

• Bachelor’s degree in social work, human services, or related field.
• Experience with seniors and/or disabled adults, case management, and strong interviewing/written communication skills.
• Ability to use computers, software, and mobile technology.

Responsibilities

• Advise veterans and caregivers on services to support aging in place.
• Screen and complete assessments and develop plans of care for veterans in NC and surrounding areas.
• Travel frequently using a personal vehicle; mileage reimbursed at IRS rate.
