Options Counselor - VDC Program - Remote Position, DC/MD

Peninsula Agency on Aging (PAA) is currentlyseeking an individual to serve as the Options Counselor for the Veteran’sDirected Care (VDC) Program to provide case management and service coordinationfor eligible VAMC clients in MD, DC, and VA.

Key Responsibilities

As the Options Counselor for the VDC Program, youwill be instrumental in advising veterans and/or their caregivers of theservices and resources available to support aging in place, and utilizeinterview skills to screen and complete assessments, provide optionscounseling, and develop comprehensive plans of care/service plans for veterans.

EssentialSkills and Experience

We are looking for applicant with anycombination of education and experience equivalent to a Bachelor’s Degree fromaccredited college or university in social work, human services, or relatedfield. Applicant needs to be Maryland, DC, Northern Virginia resident, experienced with working with veterans and/ordisabled adults, case management, excellent interviewing, oral, and writtencommunication skills, and effective use of computers, software, and mobiletechnology. Applicants must have a valid driver's license/insurance, use of personal vehicle, and willing to travel to various areas in Maryland, DC, and Virginia. A licensed Notary preferred or willingness to become one. Applicants must have theability to model our values, and get vaccinated as recommended.
