Job summary

Peninsula Agency on Aging, Inc. is seeking an Options Counselor for the VDC Program to provide case management and service coordination for eligible Veterans Directed Care clients in MD, DC, and VA. The role supports aging in place, with emphasis on interviewing, assessments, and developing care plans.

Remote position with a strong remote work culture; requires a valid driver’s license and willingness to travel within service areas. Bachelor’s degree in social work or related field preferred.

Qualifications

• Bachelor’s Degree from an accredited college or university in social work, human services, or related field.
• Maryland/DC/ Northern Virginia resident; veteran or disabled adult experience preferred.
• Excellent interviewing, oral and written communication skills; proficient with computers, software, and mobile technology.

Responsibilities

• Advise veterans and caregivers on services and resources to support aging in place.
• Screen and complete assessments; provide options counseling.
• Develop comprehensive plans of care/service plans for veterans.
