Job summary

Peninsula Agency on Aging seeks an Options Counselor for the Veterans Directed Care (VDC) Program. This remote role supports eligible veterans in MD, DC, and VA with service coordination and case management in a flexible, mission-driven environment.

The ideal candidate has a Bachelor’s degree in social work or related field, strong interview and communication skills, and ability to travel regionally when needed. Notary preferred; remote work culture is encouraged.

Qualifications

• Bachelor’s Degree in social work, human services, or related field.
• Experience with veterans and/or disabled adults and case management.
• Strong interviewing, oral, and written communication skills.

Responsibilities

• Advise veterans and caregivers on services and resources for aging in place.
• Screen and complete assessments, provide options counseling.
• Develop comprehensive plans of care/service plans for veterans.
