Job summary

Bay Aging is seeking an experienced Options Counselor to provide person-centered counseling to Veterans enrolled in the Veterans Directed Care program. This role identifies needs, develops spending plans, and coordinates supports to help Veterans remain in their homes.

The position involves in-home visits, travel, and remote work as appropriate, requires 3+ years of related experience, a Bachelor’s degree in social work or a related field, and strong communication skills.

Qualifications

• 3+ years of experience working with seniors, disabled adults, human services or general case management.
• Bachelor’s degree in social work, human services or medical field preferred.
• Criminal history background check required.

Responsibilities

• Home visits (50% including travel time) and development of a spending plan; facilitate employer processes and scheduling of required contacts and reassessments.
• Office work (50%): follow up on referrals, maintain confidential client information, perform fiscal duties, and manage satellite office administration.
