Peninsula Agency on Aging, Inc. is seeking an Options Counselor for the Veteran’s Directed Care (VDC) Program to provide case management for eligible veterans in North Carolina and surrounding areas.

The role includes interviewing, assessments, counseling, and developing comprehensive care plans, with significant travel using a personal vehicle; IRS mileage rate reimbursed. Applicants should hold a Bachelor’s Degree in social work or related field, have experience with seniors or disabled

Qualifications

• Bachelor's Degree in social work or related field.
• Experience with seniors and/or disabled adults.
• Vaccinated for COVID-19 or as required by policy.

Responsibilities

• Provide case management and service coordination for eligible clients.
• Screen and complete assessments for veterans in NC and surrounding areas.
• Provide options counseling and develop comprehensive care plans for veterans.
